For a Shirt Sponsor, please please send this sponsor form and check for $100 made out to "The Jimmy Fund".  Your business name will appear on the back of the 300+ shirts worn by youth players and HS volunteers. The name will also be listed on the tourney website.

For a Basket Sponsor, please send this sponsor form and check for $50 made out to "The Jimmy Fund". Your business name will appear on signs at all three gyms the day of the tourney and your business name listed on the tourney website.
